Community Specialist (Promotor/a) Reports To: Director of Policy and Organizing.

Position Summary:

Promotor/as are responsible for conducting Medi‑Cal outreach to build relationships with the East San Jose community and promote awareness of relevant topics and resources. They increase access to services and opportunities for community organizing and advocacy, bringing needed services into families’ lives. As respected community members, Promotor/as strengthen the ability of immigrant communities in East San Jose to identify issues of concern, confront barriers, propose locally defined solutions, inform policies, and improve individual and community health outcomes.

Essential Responsibilities:

Provide community education and outreach related to Medi‑Cal to individuals and families within designated areas of East San Jose.

Actively participate in trainings provided by Amigos de Guadalupe staff.

Provide peer resource and health education through community engagement to assess client needs and concerns as they relate to family, community, and health.

Make program referrals to Amigos de Guadalupe and external agencies, and follow up with individuals to ensure they receive needed services.

Reduce barriers to initiating or continuing to receive support by providing good information to both community members and providers.

Develop relationships with local health care and social service providers to facilitate information sharing and service provision.

Represent the organization in meetings with community partners.

Attend conferences, workshops, and trainings as directed and/or required.

Assist in completing other program goals and requirements.

Other duties as assigned.
