Nurse Practitioner or Physician Assistant - Housecalls - State Wide Traveler for IN $25,000 Student Loan Repayment or $20,000 Sign‑on Bonus for individuals who have not previously participated in this program.

Optum is seeking a Nurse Practitioner or Physician Assistant to join our HouseCalls team in Indiana. Optum is a clinician‑led care organization that is creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you will provide annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow‑up care to improve health and well‑being.

Our holistic approach addresses the physical, mental and social needs of patients wherever they may be, helping patients access and navigate care anytime and anywhere. We offer flexible scheduling with no on‑call, no weekend and no holiday requirements. The role requires 85% travel across Indiana; all travel expenses are covered. Base salary plus multiple incentives are available.

Primary Responsibilities

Conduct in‑home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.

Perform evidence‑based practice screenings, including point‑of‑care testing as appropriate.

Identify diagnoses for care management and active medical management.

Formulate a list of current and past medical conditions using clinical knowledge and assessment findings.

Communicate findings to the member’s PCP regarding potential gaps in care.

Identify urgent and emergent situations and intervene appropriately.

Educate members on disease process, medication, and compliance.

Required Qualifications

Active NP license or ability to obtain by start date; license must be unencumbered, free of open disciplinary actions, probation or restrictions.

NPs: In jurisdictions that authorize autonomous practice, must have approval to practice autonomously or without formal supervision; must obtain such approval within 1 month of hire. PAs must have a collaborative agreement in applicable states.

Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Adult‑Gerontology Acute Care; or ability to obtain by start date.

Ability to complete physical requirements: moving a 30‑pound bag, navigating stairs and various dwelling conditions, remaining in a stationary position.

Reliable transportation to travel up to 85% within licensed states.

Preferred Qualifications

Additional licensure in other geographic areas.

1+ year of clinical experience at the highest level of education.

Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.

Home health care or home visit experience.

For employees in an active regular status: must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive full student loan repayment benefits.

Pay ranges from $89,900 to $160,600 annually for full‑time employment. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. All benefits are subject to eligibility requirements. We comply with all minimum wage laws as applicable.
