Overview
The Hospice Admission Nurse is responsible for conducting initial assessments and creating personalized care plans for patients and their families. This role involves collaborating with an Interdisciplinary Group to manage patients' physical and psychological needs effectively. We aim to provide compassionate, one-on-one care while supporting a healthy work-life balance for our staff. We are now hiring an Admission RN within our Hospice division for a full-time position. Complete initial assessments of the patient’s physical status and the psycho-social status of the patient/family Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician Provide education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assess the patient/family’s response to intervention and treatment Ensure that equipment, supplies, drugs and biologicals, and other necessary items and services are available Make necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change Complete accurate and timely documentation of patient/family services provided Regularly attends IDG meetings Hospice Nurse Requirements
Registered Nurse with active, unrestricted license in the State of Michigan Community health and a wide range of Medical-Surgical experience (preferred) Reliable transportation with a valid state driver's license and current car insurance Interim HealthCare is an Equal Opportunity Employer.
