About the Role: A community-based PACE (Program of All-Inclusive Care for the Elderly) program in

Pensacola, FL

is seeking a

full-time Medical Director

to lead and oversee clinical care for a frail and elderly population. This leadership opportunity is ideal for a Board-Certified physician in

Internal Medicine or Family Medicine

with strong experience or training in

Geriatrics . In this role, you will work with an

interdisciplinary team

to coordinate care that allows seniors to maintain independence and quality of life in their own homes—avoiding institutionalization whenever possible. You’ll also participate in strategic planning, quality initiatives, and clinical supervision of providers and advanced practice clinicians.

Key Responsibilities Conduct initial, periodic, and event-based physical assessments. Develop and manage individualized medical care plans for participants. Participate in intake evaluations and interdisciplinary team meetings. Supervise care delivered by Primary Care Providers (PCPs) and Advanced Practice Providers (APPs). Educate participants and families on chronic conditions and preventive care. Collaborate with nursing and therapy staff to ensure holistic participant care. Lead and participate in Quality Assurance/Performance Improvement (QAPI) activities. Participate in on-call rotation as scheduled.

Schedule Monday – Friday, 8:00 AM – 5:00 PM On-call responsibilities as per rotation schedule No regular weekend shifts

Minimum Qualifications Active and unrestricted

Florida plenary physician license Board Certification

in

Internal Medicine

or

Family Medicine CDS certification

and

DEA registration At least 1 year

of clinical experience working with a

frail or elderly population Supervisory experience

in a healthcare setting Proficient in chronic disease management and physical assessment for older adults Strong desire to work in an

innovative and team-based care model Ability to adapt and respond to evolving patient needs

Preferred Qualifications Board Certification in Geriatrics Additional training or certification in

Geriatric Medicine Experience in a

PACE program , long-term care, or similar care models
