About PACT:

The Program for Assertive Community Treatment (PACT) is an evidence-based approach to mental health treatment using a highly collaborative model to support individuals with severe and persistent mental illnesses. DESC PACT emphasizes integrated support services for medication, therapy, social support, employment and housing. Team scheduling reflects a commitment to intensive wrap-around, 24/7 services at homes and in community settings. Team members provide rotating coverage for 12 hours per day Monday to Friday and 8 hours per day on weekends and holidays, plus on-call rotations for 24-hour crisis coverage. Major Job Responsibilities: Manage and facilitate PACT’s SUD Programming. The PACT model requires the SUDP to divide time into approximately 80% facilitation of the SUD program and 20% direct case management work. Update substance use treatment plans monthly based on client’s stage of change and propose evidence-based interventions. Act as the PACT team’s subject matter expert for substance use disorders. Provide consultation to other team members on SUD issues, treatment, and education, including Harm Reduction best practices. Provide staff training on substance use issues, as appropriate. Provide individual and group SUD treatment in office and community settings using a stage-based, non-confrontational model aligned with client goals; develop supports and treatment plans with client participation. Plan, organize and facilitate treatment and support groups for clients, as appropriate. Monitor and coordinate replacement of PACT harm reduction supplies. Advocate for client access to community resources and services; collaborate with community providers to ensure continuity of care. Develop and maintain collaborative relationships with DESC staff and external organizations to ensure full continuity of care. Participate in psychiatric consultation, supervision, team meetings and trainings; participate in clinical reviews and case conferences. Comply with the agency’s clinical accountability policies; maintain current clinical records; participate in quality assurance reviews. Comply with data gathering and program research/evaluation procedures. Participate in verbal de-escalation in emergent situations and assist staff to maintain a safe environment. Assist in unit mitigation planning and activities to support clients in maintaining healthy living conditions; coordinate with housing staff and document barriers. Collaborate with the DESC SUD team via weekly meetings and monthly consultations with SUD Clinical Supervisor and SUD Senior Program Manager. This is a multidisciplinary role, serving clients with dual diagnoses. Other duties as assigned. Specialty Area: Facilitate PACT’s SUD program as described above. Complete and update monthly substance use assessments for all PACT-enrolled clients. Prepare assessments for inpatient treatment programs. Coordinate inpatient treatment referrals through King County. Be comfortable with Harm Reduction and Motivational Interviewing approaches. Living Conditions:

Support clients in achieving and maintaining healthy living conditions, including attending care conferences, outreach, and coordinating with housing staff to maintain living environments. Other: Participate in daily morning meetings, psychiatric consultations, clinical supervision, program meetings and trainings; participate in clinical reviews and case conferences. Comply with clinical accountability policies; maintain timely and complete clinical records; participate in quality assurance reviews. Comply with program research and evaluation procedures. Other duties as assigned. Requirements MINIMUM QUALIFICATIONS: Licensed by the State of Washington as a Substance Use Disorder Professional (SUDP) in good standing. Active SUDP license issued by the Washington State Department of Health. Ability to drive to conduct agency-related business; WA driver’s license and insurable driving record required. Interest or experience engaging challenging clients in traditional mental health or SUD programs. Experience working with adults who are homeless, have mental illness and/or co-occurring SUDs, and may be involved with the criminal legal system. Strong understanding of recovery, client partnerships, client choice, and balance between protection from harm and personal dignity. Strong knowledge of community resources and how to access them. Strong communication and writing skills. Ability to work with diverse individuals and handle a wide range of behaviors. Commitment to cooperation and continuity across programs and respect for clients. PREFERRED QUALIFICATIONS: Associates degree or higher in a relevant social science. Demonstrated empathy, positive engagement with all people. Value meaningful client engagement. Potential to acquire knowledge and skills of an effective SUD Counselor. Non-judgmental approach to sensitive issues. Ability to accept feedback and work in collaborative, sometimes high-stress environments. PHYSICAL DEMANDS:
