What You’ll Do
Collaborate with a multidisciplinary team to manage patient populations with chronic health conditions.
Assess patients and develop individualized care plans for the care coordination program.
Implement and evaluate care plans to ensure optimal health outcomes.
Educate patients on disease processes, medications, and self-care strategies.
Support patients through transitions of care to improve outcomes.
What We’re Looking For Required:
Current NC driver’s license
BLS Certificate from American Red Cross or American Heart Association
Preferred:
Two (2) years of experience in primary healthcare
Ambulatory care and/or home health care within the past five (5) years.
Evidence of continuing education in health care. An equivalent combination of related education and experience may be considered.
