Screen and evaluate patients in short-term and long-term rehab, outpatient opportunities Use standardized assessments and evidence-based practices Provide rehabilitative, skilled, and medically necessary treatments Administer interventions based on individual patient needs Assess patient progress and adjust treatments accordingly Consult and counsel patients, families, caregivers, and other providers Communicate effectively with the clinical team regarding patient progress Provide comprehensive discharge summaries Qualifications:
