What You’ll Do
Provides care, treatment, counseling, and health education to patients in collaboration with Clinical Dietitians and under the direction of the Health Education Coaching Director
Provides appropriate education and training for patients ranging from pediatric to geriatric stages of life and of all educational levels
Provides services to inpatients as well as outpatients
Assists in providing diabetes skills training and the prevention of complications
Provides individualized training and teaching to individuals and groups
Responsibilities also include Diabetes Education
Provides care, treatment, counseling, and diabetes education to patients
Provides individualized diabetes education to patients from pediatric to geriatric stages of life; and for all education levels
Assists in teaching diabetes skills training and the prevention of complications
Provides individualized diabetes self-management education/training to individuals and groups in alignment with the Association of Diabetes Care & Education Specialists (ADCES) Guidelines for Practice
Performs a variety of tasks and other duties as assigned
Who We’re Looking For
Bachelor’s or Associate degree in a related field preferred.
Valid registration as a Registered Nurse in the State of Colorado required.
Certified Diabetes Care and Education Specialist (CDCES) status with the National Certification Board of Diabetes Educators required within 1 year of beginning the role or mentorship under accredited clinic. Must continue professional education per the Professional Development Portfolio of CDCES and retain certification.
Must have current BLS card within 3 months of initial employment and maintain current status throughout GRHD employment.
Minimum of two years’ professional experience in a clinical or outpatient setting in a similar role preferred.
What We Offer Our total rewards package includes
Pay Range:
$40.00- $45.73/hour (based on experience)
Shift Differentials: $2–$4/hour for nights and weekends
