JOB SUMMARY The Transitions of Care Clinical Staff Pharmacist is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently. The pharmacist will review the medications prescribed for appropriate dosing, frequency, and indications and address any discrepancies or recommendations with the patient's primary team or prescriber. The pharmacist will provide counseling on their medication regimen and appropriate resources to educate the patient about medication instructions, uses, side effects, and additional medication therapy management considerations. The pharmacist may also participate in specialty consults such as anticoagulation counseling, heart failure counseling, outpatient parenteral antimicrobial therapy and home infusion review, solid organ transplant and left ventricular assist device (LVAD) education and pill box fill, and discharge counseling for high‑risk patients discharging from UT Southwestern Clements University Hospital or Zale‑Lipshy Rehab facility. There may be additional opportunities that arise as the program develops.

EXPERIENCE AND EDUCATION Required
Doctor of Pharmacy (PharmD) degree or equivalent from an ACPE‑accredited school of pharmacy
Good general health and stress‑coping ability
Doctor of Pharmacy (PHARMD) upon hire
Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) upon hire
BLS certification with a course accredited by the American Heart Association (AHA) upon hire
Preferred
Completion of a PGY‑1 Pharmacy Practice Residency from an ASHP‑accredited residency program
2 years or more of clinical pharmacist experience
JOB DUTIES
Perform job responsibilities per organizational and departmental policies, procedures, performance improvement objectives, environmental/infection control standards, and state and federal laws and regulations.
Perform all computer order entries accurately, thoroughly, and timely. Review/update medication profiles (therapies, allergies, height, weight, etc.) and document recommendations appropriately.
Review medication orders, prescriptions, and home infusions for therapeutic appropriateness, duplications, interactions, overall effectiveness, and safety. Seek clarification if necessary and document interventions or recommendations.
Consistently work (multi‑task) and solve problems independently and effectively with minimal supervision. Take charge when necessary and be looked to for direction. Use good judgment. Be cross‑trained and multifunctional, maintaining competency in multiple areas.
Participate in cost‑effective use and selection of pharmaceuticals.
Communicate drug information verbally and in writing to physicians, nurses, etc., courteously and professionally. Assist in patient/family education and counseling regarding drug therapy and side effects. Document clinical activities, demonstrating cost savings and concisely conveying critical information.
Assist in supporting pharmacy technicians (e.g., medication history technicians) with any clinical questions and/or guidance that may be needed to complete their tasks.
Identify opportunities for improvement in the patient transitions of care process.
Actively enhance personal growth and development by participating in educational programs, in‑services, and meetings. Participate in new employee orientation and training. Take on additional projects as needs develop.
Display openness to variety within the work environment and consistently respectfully deal with all customers. Support teamwork and cooperation with co‑workers. Volunteer assistance to help co‑workers complete tasks.
Support clinical program. Support P&T initiatives by reviewing patients on high‑risk medications, collecting applicable data/measures, and communicating opportunities for system or team improvement. Participate in committees or workgroups as assigned.
Communicate effectively with patients regarding high‑risk medications and provide a comprehensive medication management review for patients at high risk for readmission.
Provide educational materials and in‑service for medical, nursing, and pharmacy team members.
Assist with Joint Commission standards and pharmacy safety initiatives.
