Job Description

As a Community Training Home II Coordinator (CTH II Coordinator) you will manage the operations of four Community Training Homes. This includes developing, supervising, and implementing each individual’s Active Treatment Plan (ATP) to ensure intensive and effective training for each client. Duties And Responsibilities

Supervise and monitor operations in each Community Training Home, maintain records, monitor client and home progress monthly, manage purchasing and inventory, and review staff logs and data to ensure follow‑up is carried out. Develop and implement training plans based on compiled progress reports to increase skills and independent living, and to encourage positive and appropriate behaviors. Schedule and conduct team meetings with staff concerning individuals’ service plans, distribute updated written plans, and ensure progress reports are recorded monthly by service providers. Ensure thorough in‑service training is provided to staff and supervise, evaluate, and train staff on maintaining dignity and privacy for individuals. Advocate for individuals by actively pursuing appropriate program levels and arranging appointments or trips for personal needs such as haircuts or clothing. Respond to facility emergencies; ensure annual inspections and safety protocols are up to date and completed timely (fire, sanitation, HVAC, etc.). Ensure daily activities are carried out, minimum coverage is met in each home, and transport individuals and administer medications if needed. Required Qualifications

Bachelor’s Degree in Human Services or Associate’s Degree with at least 2 years of related experience. One year of experience working with individuals with disabilities and special needs. Valid South Carolina Driver’s License. Preferred Qualifications

Computer skills. Management or coordinating experience. Clear oral and written communication skills. Ability to care and advocate for others. Ability to stand or walk for a long period of time. Salary: $19.00 per hour
