Job Scope This is a clinical position requiring in-depth knowledge of mental health, DSM 5-TR diagnostic criteria, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.

Essential Functions

Conduct daily visits in the community, and documenting progress notes and other required documentation in accordance with agency and OMH standards.

Complete wellness plans, comprehensive assessments, risk assessments and other documentation as required.

Provide individual psychotherapy with consumers using evidence-based treatment modalities.

Participate in daily morning meeting, inform ACT team in daily meetings and weekly clinical meetings around family issues, and incorporate findings into service plans.

Involve family and significant others in the consumer’s treatment with the ACT team and provide psycho‑education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits.

Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports.

Provide 24‑hour crisis intervention on‑call services on rotating basis.

Perform other duties consistent with the goals and objectives of the program, as directed by Team Leader.

Qualifications

Master’s Degree required – LMHC, LMSW, LCSW, or equivalent in licensure in NYS.

At least one year of experience working in human services with individuals with serious mental illness and experience working with families.

Must be outcome oriented in a fast paced environment with deadlines.
