EXPERIENCE AND EDUCATION Required

Education: Associate's Degree in Nursing

Experience: 1 year of RN experience. Specialty areas may have increased experience requirements as dictated by unit or standard of care.

Licenses and Certifications: NLC Nursing Licensure Compact (BNE Current Texas or BNE Compact State) license required upon hire; BLS Basic Life Support certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) required upon hire; CWON Certified Wound Ostomy Nurse required within 3 years of hire date.

Preferred

Licenses and Certifications: CWOCN Cert Wound Ostomy Cont RN upon hire; WCC Wound Care Certified upon hire; CWS Certified Wound Specialist upon hire; CWCN Certified Wound Care Nurse upon hire.

JOB DUTIES

Performs and accurately documents physical, psychosocial, and emotional assessments for all assigned patients.

Develops and implements an appropriate plan of care for the patient, utilizing critical thinking skills in planning and prioritizing nursing care for the assigned patients.

Completes accurate and timely documentation of the interventions, conferring with all members of the health care team in a collaborative fashion, sharing information and observations of the patient and/or patient’s family.

Initiates or maintains actions to assure continuity of safe/effective care for patients between shifts, units and upon discharge.

Evaluates implemented interventions and modifies care according to the evaluation, documenting interventions and modifications to the plan of care in an accurate and timely manner.

Adheres to appropriate hospital and/or Department of Nursing policies, procedures, clinical pathways and the accepted standards of the Wound, Ostomy, Continence Nursing Board while delivering nursing care to patients.

Provides appropriate and timely answers and/or education to patient's and/or patient’s families, completing accurate and timely documentation of all education provided.

Provides supervision of and direction to unlicensed assistive personnel and nurses new to the environment, delegating duties appropriately and within legal parameters.

Assumes accountability for all actions in the workplace, reporting errors promptly and taking appropriate follow‑up action to rectify the error.

Responds to patient's and/or patient’s family complaints in an appropriate and effective manner, reporting complaints to the Clinical Nurse Manager or their designee in an accurate and timely manner, utilizing the chain of command appropriately to resolve patient, operational, or personal work‑related problems.

Participates in performance improvement activities as directed. Completes mandatory companies and education modules.

Provides education to the community and areas of the hospital that request further information in his/her area of expertise.

Performs other duties as assigned.
