InGenesis is currently seeking a
Triage Nurse (RN)
to join our team in support of our client. In this essential role, you will provide clinical assessments, patient education, triage, and care coordination services to both adult and pediatric patients. You will help guide patients to the appropriate level of care, reduce unnecessary ER visits, and promote continuity of care—supporting the clinic’s mission to deliver timely, patient-centered healthcare.
Key Responsibilities
Conduct in-person, telephone, or video-based clinical assessments to evaluate patient symptoms and determine the appropriate level of care, utilizing standardized, evidence-based triage protocols. Apply critical thinking and clinical judgment to accurately assign triage levels and make timely referrals to appropriate care settings such as primary care, urgent care, or emergency services. Document all assessments, nursing interventions, and patient interactions clearly and thoroughly in the electronic health record (EHR) in accordance with organizational and regulatory standards. Collaborate effectively with physicians, advanced practice providers, and interdisciplinary care teams to ensure timely and coordinated patient follow-up. Identify and escalate high-risk, complex, or emergent cases to clinical leadership in accordance with established protocols. Provide patient education on chronic disease management, medication adherence, preventive health practices, and available community health resources. Participate actively in staff meetings and continuous quality improvement initiatives to support best practices in triage and patient care. Stay current with clinical guidelines, triage protocols, and healthcare regulations to ensure safe, effective, and compliant patient care.
Minimum Qualifications
Current, unrestricted RN license in the State of California. Minimum 2 years of clinical nursing experience (preferably in outpatient, family medicine, urgent care, emergency, or med/surg settings). Fluency in English, as well as in either Spanish, Vietnamese, or Mandarin, is required. Strong clinical assessment, critical thinking, communication, and documentation skills. Proficiency with EHR systems (EPIC preferred) and related healthcare technologies. Ability to work independently and manage a fast-paced workload. Must have access to a secure, private workspace with reliable internet connection.
Preferred Skills
Certification in Telephone Triage or Ambulatory Care Nursing (e.g., AAACN). Experience in telephone triage or call center-based nursing care. Background in chronic disease management and/or behavioral health.
Work Environment & Schedule
Flexible scheduling may include evenings or weekends Requires ability to sit, talk, and listen for extended periods and use computer equipment
