Days/Hours:

Monday through Friday, generally 9:00am-5:00pm, with 1 remote/field day (after 90 days of employment)

The Fortune Society, Inc. (Fortune) has been working for over 57 years in service to our mission: to support successful reentry from incarceration and promote alternatives to incarceration, thus strengthening the fabric of our communities. The organization has evolved into one of the nation's preeminent reentry and justice-informed service organizations, providing formerly incarcerated people with the skills and wrap-around services needed to break the cycle of crime and incarceration and to build productive lives in their communities.

Fortune has grown steadily over the years to an agency with close to $90 million in annual budget with just under 600 staff. We anticipate that we will keep growing, both in size and depth of service. Fortune has a dual mission: both advocacy and service. The advocacy portion of our mission focuses on building a more just criminal legal system and reducing the barriers to reentry into society. The service side of Fortune has evolved to encompass a broad range of programs that started with employment and education and now includes a robust array of services, among them: alternatives to incarceration, supervised release, court advocacy, discharge planning within correctional facilities, licensed substance use and mental health treatment, a housing continuum ranging from emergency and transitional through permanent congregate and scattered site housing, assistance in obtaining benefits, a care management unit, HIV services, a food and nutrition program, and an arts program. Each service we provide is informed by the needs of the population we serve. Fortune currently serves approximately 14,000 people a year.. Our program models are recognized, both nationally and internationally, for their quality and innovation. We have locations in Brooklyn, The Bronx, Manhattan and Queens.

MASTERS GRADS: This is an excellent opportunity for new MSW graduates looking to enter the social services field and potentially move into Fortune's outpatient Treatment Service clinic or Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS) programs once licensure is received.

Client Engagement :

This person will demonstrate an ability to engage clients in treatment and build a meaningful therapeutic relationship. This includes reluctant clients with the aim of establishing regular attendance to appointments. The person will develop a stable caseload and produce billable activities.

Time Management & Personal Organization

This person will demonstrate excellent time management skills including making, keeping and/or rescheduling appointments on their own personal calendar as well as psychiatric appointments for clients on their caseload. Successful candidate will be excellent at maintaining personal organization in pursuing numerous tasks in parallel, managing their time to maximize efficiency, and will be good at completing paperwork accurately and in a timely manner. All of this requires good computer navigation skills as well as effective typing speed.

Office Skills & Documentation

This person will have to record their client interactions in the appropriate databases and therefore have competency with computer use. The successful candidate will have the ability to focus on and process all relevant elements of their assigned tasks, ensuring accuracy and thoroughness.

Essential Duties and Responsibilities

Manage complex cases, particularly in areas like psychiatric or substance use disorders, requiring more advanced problem-solving skills (HARP, HH+); Coordinate Plan of Care for clients with criminal justice histories and chronic medical and/or mental health conditions; Screen all participants assigned for eligibility into Fortune's Care Management Unit (CMU); Offer supportive services such as, peer services, medication assistance/linkages to healthcare housing screenings and assistance with the HRA 2010e Supportive Housing Application; Serve as a Team Lead for 2 - 4 Level I care managers including the monitoring of their caseload sizes, spot checking encounter notes, and providing field training; Work closely with Fortune's interdisciplinary care team including the PCP, psychiatrist, therapist, residential service providers, DHS case workers, substance abuse treatment programs, ACT teams, Social Care Network (SCN) partners, care team, and/or family members, etc., to determine appropriate service referrals; Under supervision of management, ensure that all services delivered are within the scope of the Plan of Care and that accurate records of needs/progress/regress are maintained, as per NYS DOH Medicaid guidelines; Include input from providers/client/client's family for written individualized Plans of Care and consult with family members, parole and probation as necessary; In conjunction with the client and Behavioral Health and Care Management team, identify potential barriers to care and strategies for overcoming those barriers, including those relating to medication compliance; Regularly liaise with Fortune's housing screening team to identify potential placement opportunities for eligible individuals; Complete all requisite agency and Health Home care management trainings; Perform other duties as assigned Qualifications

Qualifications:

Bachelor's degree preferred, however candidates with a high school diploma or GED with at least 2 years of relevant experience in healthcare, social work, or mental health field will be considered. Candidates with strong college internship or field experience will also be considered; Ability to demonstrate strong written and verbal communication skills through reports, correspondence and presentations; Strong organizational and interpersonal skills; ability to manage competing priorities within a growing unit; Proficiency in MS Office, SharePoint, and Excel (working knowledge of MAPP, ePaces, PSYCKES and Foot Hold plus, SPARS); Bilingual English/Spanish strongly preferred to best suit the clients who reside in this area.
