YHDP Housing Navigator

CPSEMO is seeking a passionate and dedicated YHDP Housing Navigator to join our team. This role involves providing trauma-informed, community-based services, case management, and housing support to youth experiencing housing instability or homelessness up to their 24th birthday. The Housing Navigator will assist clients in securing stable housing, connecting with essential resources, and coordinating supportive services.
Responsibilities:
Support the successful operation of CPSEMO's youth housing initiatives under leadership guidance. Maintain and implement a low-barrier access point for youth seeking housing. Provide hands-on case management and create individualized plans to help youth achieve stable housing. Build and maintain strong community partnerships to enhance service delivery. Utilize a Housing First approach to meet clients' needs. Maintain accurate client records in databases such as HMIS. Provide transportation for clients when necessary. Organize outreach activities, including distributing program brochures and presenting at community events. Attend required trainings, meetings, and professional development opportunities. Maintain open communication with supervisors and actively contribute as a team member. Other duties as assigned. Qualifications:
Bachelor's degree in social work, psychology, public administration, or a related field (preferred). Experience working with at-risk youth populations; lived experience with housing instability preferred. Strong interpersonal and communication skills with a commitment to advocacy and support for youth. Ability to collaborate in diverse, multidisciplinary teams. Missouri Driver's License and reliable transportation with insurance. Compensation & Benefits:
