One (1) year base period with up to five (5) option years. Schedule:

Full-Time; Monday-Friday, 8:00 AM-4:30 PM (no holidays or weekends). Requirements: Master's degree in behavioral sciences. Active NPI registration number. Prior social work experience in a healthcare facility. California licensure as an LCSW, LMFT, or LPCC. Proficient in basic computer word processing (Microsoft Office, Excel). Minimum one (1) year of inpatient case management experience. Knowledge of human behavior and its impact on Veterans and families. Familiarity with medical and psychiatric diagnostic terminology. Preferred experience in mental health assessments, community outreach, and substance abuse. Understanding of VAGLAHS's social service role, housing-first model, harm reduction, and homeless services. Current California driver's license required for community service visits. Active BLS Certification. Must possess a valid driver's license and Real ID. Duties and Responsibilities: Provide services to Veterans, determining eligibility for VASH and improving communication skills. Utilize an interdisciplinary approach in service provision. Consult with VA's HUD-VASH leads on complex and ethical matters. Assist Veterans in crises, including homelessness and substance abuse, and address social and environmental challenges. Help Veterans locate suitable housing and maintain an inventory of available units accepting rental subsidies. Assist with rental applications, communication with landlords, and organize housing searches. Document housing-related advice in required systems like CPRS and HOMES. Participate in meetings to evaluate and improve Veteran services. Conduct assessments, treatment plans, and report any suspected abuse as required. Assist in the placement of Veterans in various community programs and maintain permanent housing. Support Veterans and families in addressing psychosocial needs and challenges. Maintain updated resource files for community services and document care activities as per VA policies. Complete incident reports and inquiries as directed.
