ACT Team Licensed Clinician | Masters Level Therapist

Join a dedicated Assertive Community Treatment (ACT) Team providing comprehensive support to individuals with severe and persistent mental illness who have not benefited from traditional clinic services. This role involves outreach, enrollment, and delivering personalized mental health and social services, including wellness self-management counseling, integrated dual disorder treatment, and daily living skills support. Services are primarily community-based, fostering a supportive environment where clients feel comfortable.
Education:
A Master's Degree in Social Work, Psychology, or Marriage and Family Therapy is required.
Experience:
Preferred candidates have three to five years of experience working with severe mental illness or serious emotional disturbance populations, including diverse and high-risk groups. Experience integrating care with other providers is highly valued.
Licensure and Certification:
Must hold a BSRB license at the master's level as a clinician and complete all necessary trainings for specialized care, including Qualified Mental Health Professional (QMHP) certification. A valid driver's license is required. Within six months of employment, obtain temporary Masters Addiction Counselor licensure, and within twelve months, secure full licensure. Clinical level licensure by BSRB and Clinical Addictions Counselor licensure are preferred.
Responsibilities:
Participate in daily team meetings to assign cases and review responsibilities. Conduct comprehensive assessments and develop individualized treatment and crisis prevention plans. Collaborate with community partners, caregivers, and other providers to coordinate care. Support clients in achieving treatment goals, managing crises, and maintaining safety. Engage in discharge planning and facilitate transitions to community or higher levels of care. Provide culturally competent, person-centered services and maintain timely, accurate documentation. The schedule includes varied hours, some evenings, one weekend day, and on-call rotation to support clients during crises.
