As the Community Resource Navigator , you will play a pivotal role in individuals' lives, making a significant impact through:
Receive and respond to phone calls, messages, and online referrals from individuals and families in need of assistance, inclusive of referrals from other community service providers
Completes intakes, initial assessments, referrals and education, advocacy, budgeting, screening, and provides material goods and financial assistance and documents finds disbursed as appropriate
Assess client needs and assist with negotiating systems to obtain entitlement services as needed to enhance self-sufficiency
Assists in the promotion of Catholic Charities through the attendance of community events, speaking opportunities, serving on local committees as requested by supervisor
Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver using the approved ACH screening.

Community Resource Navigator Maximum Salary: $20.00
Community Resource Navigator Schedule: Monday - Friday 8:30 -4:00
Qualifications:
High School Diploma or GED
With 3-5 years of experience
Associate's Degree in Human Services or a related field
With 1-3 years of experience
Experience working with a diverse population
