The High Risk Clinical Dietitian is responsible for practicing as an expert in the application of the Nutrition Care Process (NCP) and theory within pediatrics. The APD provides comprehensive medical nutrition services to pediatric patients and their families of Driscoll Children's Hospital through the following duties and responsibilities. This position operates within the framework of Driscoll Health System mission, vision and values.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Maintains utmost level of confidentiality at all times. Adheres to hospital policies and procedures. Complies with all regulatory standards, including federal, state and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies. Works with medical staff and nursing staff to promote optimal patient care; attends regular weekly patient care meetings on assigned patient care units. Provides coverage needs for Dietitian team in their absence or fluctuations in patient volumes in all High Risk locations. Attends and participates in departmental meetings, staff development, and professional programs, as appropriate or assigned. Serves as an active member on multidisciplinary teams. Provides medical nutrition therapy (MNT) using evidence-based practice to optimize patient care and document all pertinent clinical findings utilizing the Nutrition Care Process (NCP). Consults with health care team to ensure efficient and effective nutrition care of patients. Coordinates continuum of care with appropriate staff. Demonstrates proficiency in accordance with department and facility Standards of Care while providing age appropriate plan of care for patients and families. Conducts practice based on the most recent edition of the Code of Ethics, The Scope of Practice in Nutrition and Dietetics, the Scope of Practice for RDs, Standards of Practice (SOP) in Nutrition Care, Standards of Professional Performance (SOPP) for RDNs, and the SOP and SOPP for RDNs in Pediatric Nutrition from The Academy of Nutrition and Dietetics (Academy). Utilizes Academy standards in daily practice to consistently improve and appropriately demonstrate competency and value as a provider of safe and effective pediatric nutrition and dietetics care and service. Completes chart reviews to collect additional data that may affect the patient's health condition or nutritional risk and communicates findings to health care staff as appropriate. Able to accurately obtain anthropometric measurements. Reviews nutrition regimen to verify calculations, measurements, and specifications are accurate and appropriate. Evaluates the nutritional status of patients, including effectiveness of enteral and parenteral nutrition support regimes. Formulate and implement an individualized nutrition care plan that leads to positive outcomes. Assures implementation and evaluation of care plans by collecting additional data to support the monitoring and evaluation of nutrition intervention and educational needs. Formulates recommendations for adjustments to improve the patient's recovery and/or support his/her health condition. Monitors patients' tolerance to treatment modalities and progress through checking patient understanding and compliance with plan; determines if the plan is being implemented as prescribed; Provides and gathers information indicating reasons for lack of progress and supports conclusions with evidence. Evaluates outcomes by comparing current finding with previous status, intervention goals, and or reference standards. Serves as an educator and resource to the medical staff and allied health personnel on nutrition issues, including nutritional support therapy principles, content, and issues as needed for specific patient populations. Educates patients, families, health care team members, and members of the community. Develops, evaluates and/or revises standardized protocols and education materials for nutrition services. Processes charges in electronic medical record system as appropriate. Documents relevant, accurate, and timely information in the medical chart in accordance with the department standards. Completes all nutrition related forms (WIC scripts, DME, etc.) Maintains the formula room in accordance with hospital regulatory standards to include stocking, inventory, formula log, requesting and distributing samples etc. Responding to nutrition related patient calls in a timely manner or as per department standards. Coordinates with Director on all aspects of patient care. Monitors the departments Quality Assurance and/or Performance Improvement Programs and assists the department director in initiating corrective action. Participates in ongoing process improvement activities within the department and facility. Maintains professional growth and development through completion of required department, facility, and Academy competencies. Preforms miscellaneous job-related duties as assigned by the immediate supervisor and/or hospital administration as required. Positions may require travel (by automobile or airplane) to other clinic locations. No overnight stays. EDUCATION AND/OR EXPERIENCE:
Clinical Dietitian - Level I
Bachelor's degree or higher in nutrition or related field granted by a college or university accredited by a U.S. Department of Education (USDE) recognized accrediting agency or foreign equivalent, and coursework through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited Didactic (DPD), Coordinated (CP), Graduate Program (GP) or Foreign (FDE) program. Successful completion of Dietetic Internship Program approved by Accreditation Council for Education in Nutrition and Dietetics (ACEND). Has met the didactic and supervised practice requirements to write the registration examination for dietitian nutritionist, has Verification Statement. Eligible for registration. Pediatric experience preferred not required. If considered for Dialysis services minimum of one (1) year experience with valid RD and LD credentials post Dietetic Internship, required. Zero to 6 years' experience in nutrition care, education, counseling, private practice, community/public health, school system, long-term care or related field with valid NDTR credential or other professional designation. Bilingual (Spanish): Ability to speak and write Spanish highly preferred. Clinical Dietitian - Level II
Bachelor's degree or higher in nutrition or related field granted by a college or university accredited by a U.S. Department of Education (USDE) recognized accrediting agency or foreign equivalent, and coursework through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited Didactic (DPD), Coordinated (CP), Graduate Program (GP) or Foreign (FDE) program. Master's degree in nutrition, dietetics, public health, education, health care administration, or related field preferred not required. ā„ 3 years (6,240 hours) direct clinical care for a specific age population or specialty clinic or general adult care with valid RD credentials. Example: care provided to a pediatric inpatient oncology unit, an outpatient clinic for patients under 5 years of age, or bariatric clinic. May consider up to 2 years equivalent combination of advanced education and professional experience in nutrition or related field with valid RD, NDTR or other professional designation. Bilingual (Spanish): Ability to speak and write Spanish highly preferred. CERTIFICATES, LICENSES, REGISTRATIONS:
Current Registered Dietitian or Registered Dietitian Nutritionist Certification by the Commission on Dietetic Registration (CDR). State licensure by the Texas Department of Licensing & Regulation (TDLR) must be obtained with 90 days of hire. Food Manager's Certification or Food Safety Certification must be obtained with 90 days of hire. Basic Life Skills (BLS) Certification must be obtained with 90 days of hire. Must have a valid unrestricted driver's license. Preferred for Level II:
An expert RDN possesses CDR Advanced Practitioner Certification in Clinical Nutrition (RDN-AP), and/or specialty certification(s) from a nationally recognized certifying body, such as CDR Pediatric Nutrition Specialist (CSP), or CDR Pediatric Critical Care Nutrition (CSPCC).
