What You'll Be Doing Main Function:
The Counselor is responsible for assessing, counseling, intervening, implementing and educating the patient and family/significant others to facilitate movement throughout the continuum of care.
Makes psychosocial assessments appropriate to patient’s age and diagnoses.
Appropriately prioritizes cases according to assessed needs
Written assessments completed within 24 hours of assessment
Documented assessment demonstrates understanding of patient’s functional status and psychosocial situation
Assessments are completed based on information obtained from patients, families/significant others, staff and interdisciplinary rounds as needed
Assessment prepares for the possibility of changes in the patient’s condition/status
Provides supportive counseling to patients and families
Coordinates discharge planning services.
What We're Looking For
Bachelor’s Degree required in Counseling, Social Work or related field
0 – 2 years required
CADC/LCADC must be obtained within 3 years of date of hire.
