The primary function of this position is to connect community and social supports with health care, and provide better organization of medical and behavioral health care. A Health Home Care Manager ensures all caregivers involved with a person talk to one another so they can focus on their client's needs.

Qualifications

Education/Training: A master's degree in one of the qualifying fields and one year of experience; OR A bachelor's degree in one of the qualifying fields and two years of experience; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two years of experience; OR A bachelor's degree in ANY field with either: three years of experience, or two years of experience as a Health Home Care Manager serving the SMI or SED population.

Experience: Providing direct services to individuals with Serious Mental Illness (SMI), developmental disabilities, alcoholism or substance abuse, and/or children with SED; OR Linking individuals with these needs to a broad range of services essential to successful community living (e.g., medical, psychiatric, social, education, legal, housing, and financial services).
