POSITION SUMMARY
MLKCH is excited to invite applications for the role of Community Outreach Coordinator within our Street Medicine program. In this rewarding position, you will play a critical role in assisting patients in navigating community services, accessing resources, and fostering healthy behaviors. Your efforts will primarily focus on supporting individuals facing homelessness in various encampments, ensuring they receive the care and advocacy they need. We're looking for a passionate candidate who has experience connecting with and supporting individuals in these communities.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Build trust and rapport with patients through supportive interactions.
Conduct follow-ups and goal-setting sessions utilizing motivational interviewing techniques.
Engage with patients through various methods, including phone calls and home visits.
Assist patients in completing necessary applications and assessing insurance eligibility.
Help patients establish personal health goals and provide appointment guidance.
Connect patients to transportation resources and offer appointment reminders when necessary.
Enhance communication of Street Medicine's mission with patients and stakeholders.
Collaborate closely with healthcare providers to ensure comprehensive patient care.
Work amicably with other clinical team members on shared patient cases.
Stay informed about community resources relevant to patients' needs.
Maintain consistent communication with the Street Medicine team for effective patient evaluation.
Serve as a liaison between patients and community service agencies.
Document patient management details in the EMR system.
Participate in regular staff meetings and training sessions as needed.
Manage a caseload of approximately 15 patients.
Complete the Housing Assessment and Individualized Housing Support Plan for enrolled patients.
Identify neighborhoods for outreach and develop efficient canvassing routes.
Engage residents door-to-door to gather information and potential patient leads.
Assist in training new staff members as required.
Provide input on team safety while in the field and address any concerns from team members.
Perform other related duties as assigned.
POSITION REQUIREMENTS
A. Education
High School Diploma or GED equivalent.
Preferred College degree or vocational training in a health-related field.
B. Qualifications/Experience
Proven experience fostering strong relationships within homeless encampments and communities.
Familiarity with homelessness issues and promoting community relations.
Current Basic Life Support (BLS) certification from the American Heart Association.
Completion of Workplace Violence Prevention Program/Certificate as per hospital policy within 30 days of hire.
Valid and unrestricted CA Driver's License.
Demonstrated ability to assess and address the needs of individuals experiencing homelessness.
Capacity to build consensus on strategies addressing homeless patients' needs.
Basic computer skills (Outlook, Word, PowerPoint, Excel); prior experience with EMR systems preferred.
Experience in a healthcare facility or Street Medicine environment is a plus.
Bilingual in Spanish is preferred.
C. Special Skills/Knowledge
Strong interpersonal skills for effective engagement with individuals living on the streets.
Understanding of the challenges faced by unsheltered homeless individuals.
Knowledge of public healthcare systems is preferred.
Lived experience of homelessness is desirable.
