Our client, a community hospital system in Glenwood Springs, CO, is seeking to hire a full-time pharmacist for their outpatient pharmacy.

We are looking for a community pharmacist that wants to be part of a team that is dedicated to supporting each other in the day-to-day operations and challenges today's community pharmacy is facing. Our mission is to improve patient outcomes both on hospital admission with medication reconciliation, on discharge with discharge medication dispensation, and in their day-to-day daily life outside the hospital.

The ideal candidate will:

Staff in the outpatient pharmacy and collaborate with interdisciplinary care teams both inside and outside the hospital to fulfill hospital discharge orders while balancing the needs of our community patients that we serve Staff rotating Medication Reconciliation Pharmacist shifts with our other outpatient pharmacy staff pharmacists Be adaptable and ready to collaborate with your pharmacy team (inpatient and outpatient) and all area healthcare providers Hours are M-F 7a - 5:30p and Saturdays 8a - 5p. They are closed Sundays

QUALIFICATIONS:

Education:

Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD Pharmacy degree. Current license to practice pharmacy in the state of Colorado or must obtain within 60 days of hire Immunization Certified with active BLS card BENEFITS
