The successful candidate:
Provides direct instruction to students with disabilities that is designed to meet their individual transition needs related to the transition Develops curriculum, designs and implements lesson plans, and assesses student progress Disseminates information to staff, students, and parents regarding student needs, progress, transition, and resources Coordinates transition activities for college, career, and community readiness Supports special education teachers in developing transition plans for IEPs Works with local businesses to develop partnerships for job shadowing, internships, job coaching and employment Facilitates referral, application, eligibility process for students and families to adult service providers Assists with alignment of transition continuum of services at the secondary level Attends IEP meetings for students in district Other duties as assigned
Bachelor's degree in Special Education; Master's degree preferred
PA Certification PK-12 or 7-12 in Special Education required
5 years teaching experience in special education preferred; Experience in high school setting preferred
Knowledge of adult service providers and adult services for special education and non-special education students
Experience in transition planning preferred Acts 34, 114, and 151 Clearances required prior to beginning work
