The Clinical Pharmacist works in a large tertiary care medical center with a primary emphasis on inpatient care, outpatient care, and pharmacy-based clinical services. The medical center is affiliated with the University of Medicine and Dentistry of New Jersey and the Rutgers College of Pharmacy. The Pharmacy has three locations at E?O (Lyons) and the James J. Howard Outpatient Clinic in Brick, NJ. Pharmacists may be asked to work at any of the sites if needed. Qualifications

Graduate of an ACPE?accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure:
Full current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (including Puerto Rico) or the District of Columbia. Maintain current registration if required for licensure; if license has been revoked, suspended, or otherwise restricted, appointment may only occur in accordance with VA Handbook 5005?II, Chapter?3, ?B, paragraph?16. Partial licensure may be waived for up to 2 years if the pharmacist is supervised by an appropriately licensed pharmacist. For grade levels above GS?11 the candidate must be licensed. GS?12 Clinical Pharmacist candidates must either possess one year of experience equivalent to the next lower grade level or have completed an ACPE?accredited Pharm.D. program. Candidates must demonstrate knowledge, skills, and abilities (KSAs) in all professional pharmacy practice areas, communication, drug regulation, monitoring, and safety. Preferred experience: Inpatient and outpatient (retail) practice.
Basic Requirements
Full current and unrestricted license to practice pharmacy in the United States. Maintain current pharmacist licensure/registration and scope of practice for the area(s) assigned. Full performance level of the vacancy is GS?12. Physical requirements: work requires long periods of sitting, standing, walking, bending, twisting, and carrying items up to 25?lbs.
Duties
Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols. Interprets medication orders (verbal, written, and electronic) and transcribes to patient medication profiles accurately. Maintains accurate and complete patient medication profiles. Accurately compounds and/or dispenses pharmaceuticals for various administration routes including oral, topical, and parenteral. Processes, dispenses, and/or distributes controlled substances while maintaining appropriate accountability and records as required by agency regulations and federal law. Ensures safe, appropriate, and cost?effective medication therapies for patients according to established policies and procedures. Reviews drug therapy regimens for drug?related problems including indications, contraindications, drug?drug interactions, drug?food interactions, allergies, and culture/susceptibility data. Provides clinical consultation, including non?formulary consultation, and clarifies medication therapy to practitioners. Supports the development, implementation, and maintenance of clinical pharmacy services and treatment guidelines. Participates in the formulation of policies, procedures, and programs related to safe medication use. Maintains competency for the current job title/position, participates in staff development, and engages in new employee orientation and training programs.
