Purpose of the Role: The Street Outreach Specialist is part of a coordinated outreach team that identifies and engages people living in unsheltered locations, such as cars, parks, abandoned buildings, encampments, and on the streets, including homeless individuals at risk of becoming homeless. The outreach team reaches people who might not otherwise seek assistance or come to the attention of the homelessness service system. Through this work, the team ensures basic needs are met and supports individuals along pathways toward housing stability and quality life improvements.
Essential Functions of the Role:
Identify and build relationships with organizations and individuals in places such as hospitals, shelters, partner agencies, medical care clinics, mental health providers, soup kitchens, food pantries, and other community resources to promote CHOICE mission and services.
Respond to calls or requests from the community (public agencies, businesses, libraries, hospitals, family members, etc.) regarding homeless individuals and those experiencing crisis.
Co‑respond to crisis calls with local government agencies.
Identify “hot spots” where individuals are most likely to be found (e.g., early morning and evenings).
Establish rapport and provide life‑saving resources such as food, water, clothing, blankets, and other necessities.
Use a trauma‑informed approach with crisis intervention, harm reduction, and culturally responsive engagement techniques.
Meet one‑on‑one with individuals needing services and screen for eligibility.
For eligible persons, remain in close partnership, monitoring enrollment until intake is completed.
Assist clients with completion of enrollment and related documentation.
Complete street outreach assessment (VI‑SPDAT) as requested by the Homeless Outreach Supervisor.
Document all outreach efforts in HMIS (Homeless Management Information System) and the Relevant and Awards database.
Attend meetings at the Department of Community Mental Health and other community events as needed.
Proactively report trends, relationships and developments in the field to the Outreach Supervisor.
Fieldwork Requirements:
Conduct daily street outreach and respond to calls for assistance.
Conduct a minimum of 20 intakes per week.
Track activities and outcomes.
Spanish speaking (required).

Pay $36,000.00 – $38,000.00 per year
