Job Summary:

The Clinical Case Manager (RN) works in collaboration with Medical, Nursing, Behavioral Health, and Pharmacy staff to actively manage plans of care for patients being treated for opioid and alcohol addiction. The RN directly supports the Medical providers prescribing treatment for substance abuse and provides case management services for patients as part of their primary care. As the primary clinical support staff person for this program, the RN is responsible for receiving referrals to treatment and enrolling new patients, scheduling and facilitation of provider visits, coordination of patient testing, oversight of patient compliance with treatment program requirements, and reporting for compliance with regulatory requirements. Key functions will include initial and ongoing patient education and support, referrals to addictions counseling, maintenance and tracking of patient panels, and regular reporting on quality measures for program success. By coordinating the efforts of Nursing, Medical, Behavioral Health, and Pharmacy staff the RN serves as the primary administrator for providers treating patients with opioid and alcohol addiction.

Representative Duties:

Under general supervision of the Senior Case Manager job incumbent will:

1. Telephone and in-person patient triage, following up on patient issues.

2. Receive and process referrals from TCC Medical staff (Primary Care, Urgent Care, and Psychiatry) as well as community partners (FMH Emergency Department).

3. Facilitate patient intake and education, coordinating treatment plans, patient consents, and program compliance agreements.

4. Making all necessary referrals to Social Work, Primary Care, Urgent Care, Behavioral Health or other TCC or community services to address patient needs in order ensure compliance with TCC requirements and facilitate successful treatment.

5. Perform medication reconciliations, drug interaction monitoring, and review of PDMP records to ensure patient safety and compliance with program requirements.

6. Coordinate referrals to non-TCC providers for higher level care (e.g. methadone treatment programs) for patients unable to remain in compliance with provider and TCC requirements.

7. Maintain a patient panel to ensure an accurate patient census and facilitate monthly audits for compliance with DEA patient limits for each provider.

8. Establish and maintain appointment schedules for all waivered medical providers and staff to ensure that patients have guaranteed access to care in order to comply with program requirements.

The RN will administer schedules for:

9. New, Restart, and Maintenance provider visits.

10. Naltrexone New, Restart, and Maintenance provider visits.

11. RN Nurse Visits, direct staff or assist providers' staff with visits to include pre-visit planning and chart review, rooming and screening patients, and assisting medical providers with patient care.

12. Collect and appropriately handle lab specimens.

13. Assist with or perform basic assessments and procedures (e.g. vital signs, ECGs, CIWA assessments, substance use and behavioral health screenings, etc.).

14. Coordinate with TCC Billing & Coding departments to ensure that all services rendered are documented and coded correctly for billing and reporting purposes.

15. Execute delegation protocols.

16. Assist with or facilitate medication management in collaboration with Pharmacy (e.g. medication refills, organizing medication boxes for patients, patient education, etc.).

17. Complete accurate and timely documentation in the electronic medical record.

18. Assist with management of urgent and emergent patient care situations as needed.

19. Actively work to ensure that all patient encounters provide an experience in alignment with TCC values.

20. Work cooperatively with TCC Medical and Nursing to ensure that all AAAHC standards are met and that compliance with these standards is occurring on a continual basis.

21. Develop and cultivate a strong spirit of effective teamwork amongst clinicians who share responsibility for treating patients with opioid and alcohol addiction.

Other Responsibilities:

1. Perform other job related duties as assigned. Tanana Chiefs Conference expects that all nurses will maintain the ability to perform basic clinical patient care skills. Clinical skills competencies will be demonstrated annually per the requirements of individual roles and as needed in support of general patient care services.

Minimum Qualifications

Minimum Qualifications:

1. Current Alaska licensure as a Registered Nurse in good standing. BSN or higher degree preferred.

2. Two (2) years of experience providing direct care or case management in an outpatient primary care, urgent care, or emergency room setting.

3. Experience providing nursing care or case management in a Medication Assisted Treatment (MAT) or other substance use treatment program is strongly preferred.

4. Current BLS or ability to obtain or renew prior to beginning work.

5. Experience working with Alaska Native peoples or cross-cultural experience preferred.

6. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements.

Knowledge, Skills and Abilities:

1. Knowledge and experience documenting patient care in an electronic medical record.

2. Excellent verbal, written, and presentation skills.

3. Ability to make knowledgeable clinical assessments and judgments.

4. Ability to work independently and with minimal supervision while managing a complex panel of high needs patients.

5. Ability to set, communicate, and maintain strict personal and professional boundaries patients.

6. Ability to educate staff and patients in a positive, non-demeaning manner.

7. Ability to provide counseling and emotional support with compassion and respect for the individual.

8. Ability to provide support to the members of the care team (teamwork).

9. Must possess initiative and ability to work independently.

10. Ability to maintain confidentiality of medical records.

Supervision:

This position has no direct supervisory responsibilities.

Supervision

Physical demands:

Ability to lift and carry up to twenty (20) pounds routinely and up to a maximum of fifty (50) pounds occasionally. Must be able to safely assist in patient transfers, lifts, and carries using appropriate body mechanics in coordination with other staff.

This is a high public contact position including daily interaction with patients and health services staff. Position is obligated to follow Health Center, TCC and nursing principles to ensure safety of patients and staff. Must be able to work in a team or independently. Job incumbent must promote a courteous, professional and efficient public image to patients and referral providers/entities.
