Position Summary:
The case manager will provide support and service linkages to residents within the program, coordinate services, and ensure quality care for the residents.
Essential Accountabilities:
Routinely visit apartments and assess residents’ functioning level.
Provide direct care services, including planning and implementing goals and training to enhance activities of daily living skills. These include, but are not limited to:
Budgeting
Grooming
Nutritional planning
Public transportation/travel training
Community resource/use integration
Shopping/menu planning
Housekeeping
Socialization skills
Physical health maintenance
Assertiveness/self‑advocacy training
Maintain necessary written documentation, including functional assessments, medication updates, progress notes and reports, and annual and quarterly service plans.
Screen candidates for residential placement, attend medical or treatment programs as required, and conduct 30‑day follow‑ups on discharged residents.
Assist residents in planning and coordinating recreational/social activities, escorting them to events, and participating in moderate physical activities.
Attend all staff meetings and ensure that resident meetings are scheduled promptly.
Accountable for the opening and follow‑ups of new apartments, maintenance, upkeep, and safety of assigned apartments.
Report and document all incidents within 24 hours to the immediate supervisor or Director of Behavioral Health, and maintain resident files monthly.
Responsible for crisis intervention.
Monitor environmental issues and report them to the immediate supervisor or Director of Behavioral Health.
Participate in activities of other staff members during absences or staff shortages.
Represent the agency at meetings and trainings as designated.
Qualifications:
Associate’s Degree in Human Services or a High School diploma with two (2) years of experience in the Human Services field.
New York State driver’s license and reliable vehicle access.
