Description
Position Summary
The Quality Improvement Coordinator (QIC) will facilitate collection and analysis of patient data across multiple systems and the generation of care gap reports or other tools to improve quality outcomes for NeighborHealth Center (NHC) patients. Based on established performance measures, the QIC will collect and analyze data and partner with the Quality Director to implement strategies to improve patient health and cost cost-effectiveness of care. The QIC will report on relevant quality performance measures at monthly quality improvement meetings and will work with and provide program data to clinical leadership. QIC will work with billing team to ensure proper coding for evaluation by PHP quality performance measures.

Principal Duties and Responsibilities*
Quality Improvement Projects
Coordinate quality improvement (QI) initiatives across various departments to improve healthcare services, patient outcomes, and operational processes. Identify areas of opportunity for quality improvement through data analysis and feedback from staff, patients, and in collaboration with health plans. Monitor quality improvement projects within the various departments and offer suggestions for change or adaptation.
Data Collection and Analysis
Collect, analyze, and report on data related to quality measures, including patient satisfaction, safety metrics, and clinical outcomes. Develop dashboards, reports, and presentations to communicate findings to quality committee board members as well as Quality Director and CMO monthly. Ensure the accuracy, reliability, and timeliness of quality data. Assist with annual reporting of UDS data and submission. Assist with other clinical reporting needs, as assigned.
Continuous Improvement
Evaluate the impact of QI initiatives and make recommendations for ongoing improvements. Monitor and evaluate QI initiatives in accordance with PHP incentive programs and monitor incentives. Act as EPIC Champion for QI initiatives and represent NHC with collaborative workgroups including UNC and PCIC.
Required Skills or Abilities*
Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel with keyboarding skills of at least 45 wpm. Demonstrated ability to build inclusive, respectful relationships and advocate for individuals from diverse backgrounds, fostering equity within the workplace and community. Strong personal integrity with a proactive, detail-oriented approach; able to work independently, take initiative, and align priorities with the clinic's mission. Excellent interpersonal and communication skills; able to lead, collaborate, and problem-solve effectively in a fast-paced, dynamic healthcare environment.
Required Knowledge, Experience, Or Licensure/Registration
Bachelor's degree or equivalent experience in lieu of education 2 to 4 years of experience in healthcare quality or data roles Knowledge of quality improvement frameworks, clinical quality measures, and healthcare data analysis Proficiency with data gathering and reporting and with EHRs (EPIC highly preferred) Ability to work onsite Mondays through Fridays during the hours of 8am and 5pm. Ability to read, write, speak, and comprehend English fluently. Up-to-date immunizations
