Description
The Community Outreach Coordinator is responsible for identifying, cultivating, and managing strategic partnerships with community-based organizations and other referral sources to expand client enrollment in HeartShare's Health Home program. This role requires a results-driven, outgoing, and assertive individual with the ability to perform in-field outreach, develop strong relationships, and manage a client recruitment pipeline. The coordinator will serve as a brand ambassador for HeartShare, ensuring our mission and services are represented with professionalism, urgency, and integrity.
Primary Responsibilities and Essential Functions of Position: Collaborate with the Program Director to identify and develop new referral partnerships with community-based organizations and health and social service providers Aggressively pursue and enroll a minimum of 30 eligible clients per month into the Health Home program through proactive community outreach, direct engagement, and consistent follow-up Conduct outreach and engagement through field and home visits, phone calls, and text messages to ensure client contact and completion of the enrollment process Maintain a strong tracking system for all recruitment activities including lead status, follow-ups, outreach methods, and enrollment outcomes Organize and deliver presentations to external stakeholders about HeartShare's services and enrollment process to foster partnerships and client recruitment Troubleshoot and resolve partner or client engagement issues quickly and professionally, maintaining a strong customer service ethic Monitor and adjust recruitment strategies in response to performance trends and evolving community needs Identify barriers to engagement and develop client-centered solutions that address social determinants impacting enrollment Maintain detailed knowledge of HeartShare's Health Home model and eligibility criteria Provide accurate and timely documentation and reporting, including referral logs, enrollment data, outreach summaries, and weekly/monthly status updates Adhere to all regulatory and funder guidelines Perform other tasks and initiatives within the scope of the role as assigned by the Program Director or Senior Director To recognize exceptional performance in client enrollment, the Community Outreach Coordinator will be eligible for a monthly bonus based on the number of enrollments achieved. Bonuses range from $500 for 20 enrollments to $1,000 for 30 enrollments, with incremental rewards at 22 ($600), 25 ($700), 27 ($800), and 28 ($900) enrollments.
Qualifications
High school diploma or GED
