Essential Functions of this position are:

To achieve and to maintain compliance with regulations within PA Chapters §5310; §6400 and §6500 that reference the work/service responsibilities of Program Specialists. To provide training (1) to participants upon enrollment, and annually, rights of the individual and civil rights, complaint process and grievance process, and (2) to residential staff on provisions and updates to the Individual Support Plan (ISP). Case Manager ( Functions as Program Specialist) is responsible for conducting comprehensive functional and social, emotional and environmental needs assessments; making appropriate referrals to community and treatment supports; and coordinates levels of care transition needs Support team members in addressing clinical concerns Work with team to implement person-centered effective programs for each individual Coordinate teaching and skill development for individuals and team members Complete application and monitor benefits for individuals (ex: Supplemental Habilitation and Needs Exception Assessments) Ensure compliance with regulatory body. Support individuals to become independent, make decisions, and live a meaningful dignified life with valued social roles (ex: friend, neighbor, student, employee) Coordinate referral and intake process as directed. Conduct initial and ongoing assessments of program participant's strengths, interests, and areas needing assistance. Participate in Supports Intensity Scale assessment and utilization of information gained. Coordinate and participate in PCHC reviews and complete Health Risk Screenings (HRS) Assist in the development, implementation, and monitoring of goal plans and treatment interventions. Coordinate the orientation of new program participants, take inventory of all personal items upon admission, and conduct introductions to staff and consumers. Evaluate individual's progress Coordination of some program participant appointments. Assist consumers in planning, coordination, and implementation of daily, evening, and weekend social and recreational activities. Participate in the development of individual service plans designed to improve the social and self- care skills of program clients. Assist in providing an orientation for new employees in agency and service location specific to Individual Support Plans, Behavior Plans, Individual and Supervision Protocols Assess, teach, and evaluate staff on an ongoing basis regarding their work on individual service plans. Attend monthly Support Coordination meetings when necessary. Work in conjunction with Supports Coordinators to assess and complete participant prioritization of needs (PUNS) to address any unmet needs, assess urgency of need. In conjunction with Supports Coordination, completes a periodic review of PUN's including update at least annually or whenever the individual's needs change. Assess effective use of individual's budget &utilization projected and authorized units Coordinate support planning with Supports Coordination, Behavior Supports and other team members to ensure consistency of services. Facilitate the resolution of any barriers in service delivery, health, and or welfare & documentation of closing the loop. Documentation of all services and supports provided in participant's health record or electronic health record (service notes, quarterly review, meeting minutes, monitoring tools) Serve as a liaison with outside agencies and individuals as appropriate. Advocate for continuity of services, system flexibility and integration Working knowledge of funding streams, discussion of service preference and referral process Obtain competencies in medical issues & genetic diagnosis Perform job within framework of agency, site and professional standards. Participate in staff meetings and training programs. Attend and participate in agency events. Adheres to agency policies and procedures Project a positive attitude and image to program participants, family members and outside contacts Participate in 40 hours of training per fiscal year Adhering to all funder requirements To provide on-going evaluation to monitor the necessary level of services required to promote an individual's growth.

This position requires:

Bachelor's Degree with major course work in human services, social work, psychology or Associates degree other related social sciences plus 2 years of experience working in the intellectual disabilities field and /or behavior health or related experience. Demonstrate an understanding of individuals with developmental delays and sensitivity in working with participants and families. Works with participants and significant others in a manner which demonstrates respect and concern for the person's welfare. Possess knowledge of basic principles and methods of care coordination and case management Computer competency Facilitation/team leader skills Time management skills Valid Driver's License
