Job Description Pay: $90,000 - $100,000 / annually + $10,000 bonus after 90 days Schedule: Full-time, Monday - Friday, 8 am - 5 pm. Responsibilities

The Director of Health Services (DHS) embodies Yoi Shigoto, the good work, by leading the overall implementation of health and wellness services in the community. The DHS serves as a liaison with families, hospital personnel, physicians, community organizations, and other related service agencies to provide quality care to MBK Senior Living residents. The DHS is incredibly important for setting the stage for MBK Senior Living as the senior living provider of choice. Essential duties include hiring, training, managing, and leading a care team to provide superior care, while maintaining the physical, psychological and emotional wellness of residents, and in accordance with MBK\'s core values. The independence, dignity, autonomy, and purpose needs of residents will be integrated into the plan of care as written and directed by the DHS. Residents and their loved ones will be included as partners in the development of individualized service plans. Conduct and coordinate potential resident assessments in accordance with current rules, regulations, and community policies and procedures that govern resident assessment, including: initial assessments before move-in, comprehensive care plans at move-in, and involvement of memory care director for applicable residents Draft initial service plans for new residents; review with responsible parties and Executive Director; establish service plans that promote independence and whole-person wellness Conduct routinely scheduled re-assessments and change of condition assessments; update service plans and obtain signatures per policy Ensure continuity of residents\' total wellness by coordinating health and wellness services with other resident services; work with all departments to promote quality of life Develop and maintain relationships with family and service providers; coordinate care with physicians and family members Meet regularly with wellness programming directors and the executive chef to ensure programs and diets support wellness Participate in CQI initiatives and lead interdisciplinary rounds; oversee health and wellness databases and systems Collaborate on safety, infection control, disaster preparedness, and elopement drills Recruit, interview, hire, manage, schedule, coach, train, evaluate, and supervise department staff; ensure staff training and compliance with regulations Coordinate prescription orders with doctors\' offices and pharmacies; oversee medication delivery and records Act as a Manager on Duty when necessary and attend required meetings and trainings Non-Essential Job Duties: Perform other duties or projects as assigned; collaborate with MBK groups to ensure policy compliance; maintain a safe environment; comply with regulations; promote teamwork; assist in budgeting and cost controls; Minimum Job Requirements

Certified RN, LPN, or LVN is essential or 2 years of full-time college attendance Must have completed required nursing coursework for certification; maintain license with CEU and renewal Prior management or supervisory experience required; related healthcare leadership experience preferred Current First Aid Certification; background clearances as required Health screening and negative TB test within 6 months or 7 days of employment Valid driver\'s license and insurance; reliable transportation for assessments Solid PC skills; proficiency in Microsoft Office; ability to manage documentation and records Excellent communication skills in English; basic math skills for budgeting and personnel actions Ability to make independent decisions in emergency situations; professional conduct with residents, families, and staff Able to maintain a neat, safe work environment and handle resident care coordination Preferred Job Requirements: Bachelor\'s Degree in Nursing, Human Services or Health Care field is highly desirable Physical Demands

Move intermittently throughout the day; ability to lift/carry up to 40+ pounds Push/pull residents up to 250 pounds; assist in moving residents as needed Company Information
