Our state-of-the-art 288-bed facility features a cancer center, maternity center, breast health center, two cardiac catheterization labs, and is recognized as a designated Stroke Center. In the role of Care Coordinator, you will take charge of intensive care management for adult patients, acting as a vital liaison among an interdisciplinary team that includes social workers, Physicians, Physical Therapists, and Occupational Therapists. Your leadership will ensure individualized care plans, promote optimal length of stay, and proactively address outpatient service needs to help prevent hospital readmission. This is a day shift position from Monday to Friday covering hours from 8:00 am to 4:00 pm or 9:00 am to 5:00 pm, with rotating weekend and holiday coverage. Preferred Criteria 2 years of case management or care coordination experience in an inpatient, HMO, or outpatient setting 1 year of leadership experience in a healthcare environment Masters in Nursing or a related field Required Criteria Bachelor's degree in Nursing (BSN) Current NYS Registered Nurse (RN) licensure 3 to 5 years of healthcare experience in an acute care setting Exceptional communication, problem-solving skills, business acumen, and interpersonal skills are essential Join a healthcare system that values employee engagement, and fosters a culture of respect, belonging, and inclusivity.
