POSITION SUMMARY
MLK Community Healthcare is on the lookout for a dedicated Licensed Vocational Nurse (LVN) Coordinator to join our Care Management program. In this pivotal role, you will coordinate care for patients enrolled in Enhanced Care Management (ECM) and Disease Management programs. As an integral member of the interdisciplinary care team, you will engage in effective discussions around medication management and ensure timely follow-up with primary care providers and specialists. Your clinical assessments will encompass behavioral health and social service needs, facilitating the development of tailored care plans that support patient health goals.
This dynamic position requires self-motivation, strong clinical acumen, and exceptional patient engagement skills. You will collaborate with various partner organizations to provide seamless transitional healthcare support.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Deliver clinical assistance for ECM and Disease Management, addressing both medical and behavioral health needs.
Evaluate member needs and develop actionable plans to ensure appropriate services are received, while monitoring progress towards care plan goals.
Identify healthcare needs focusing on individual health conditions and recommend appropriate interventions.
Actively address patient needs by bridging care gaps and facilitating timely follow-ups with healthcare providers.
Consult with Care Managers to review relevant medical documents and prepare for patient appointments.
Provide training to staff on documentation standards and quality measure workflows to enhance operational efficiency.
Engage with vulnerable populations as part of a multidisciplinary outreach team, including home visits and support in various settings.
Address Social Determinants of Health by connecting patients with community-based organizations.
Coordinate hospital admissions and discharges in collaboration with various stakeholders to prevent readmissions when possible.
Conduct timely medication reconciliations and support medication adherence.
Assist with billing submissions and health plan chart audits.
Travel to MLKCH ambulatory and administrative sites as necessary for patient care.
Other duties as assigned.
POSITION REQUIREMENTS
A. Education
High School graduate or GED.
B. Qualifications/Experience
Minimum of one (1) year experience in a medical practice or related healthcare environment.
Six (6) months of project coordination experience preferred.
Valid unrestricted CA Driver's License with proof of vehicle insurance is required.
Current Basic Life Support (BLS) certification from the American Heart Association.
Current Licensed Vocational Nurse (LVN) state license.
Preferred experience in Care Management.
A collaborative team player with strong organizational and communication skills.
Confidence in outreach and communication with community health care organizations.
Demonstrated sound judgment and problem-solving abilities.
C. Special Skills/Knowledge
Experience in understanding homelessness and promoting community relations is preferred.
Ability to build consensus among peers to address the needs of homeless patients.
Skilled in coordinating care with various support staff to foster a productive team environment.
Committed to providing primary care services to underserved populations, demonstrating sensitivity to diverse backgrounds.
Strong organizational and problem-solving skills, with flexibility and adaptability to change.
Ability to effectively present information to employees, community partners, and vendors.
Capacity to work independently while seeking direction on key issues.
Able to interact effectively with diverse individuals and groups.
Understanding of public health care systems is preferred.
