Your Role as a Foster Care Program Case Manager: We are looking for an enthusiastic and dedicated Foster Care Program Case Manager to oversee the critical needs of children and families within the foster care system. You will play a vital role in developing personalized treatment plans and coordinating essential services for children in foster care. Key Responsibilities: Craft and execute personalized treatment plans tailored for children in the foster care system. Maintain comprehensive and meticulous case records, providing weekly activity/status updates. Link children to necessary services, ensuring continuous access and minimizing fragmentation of care. Facilitate relationship building between birth parents, foster families, and children to promote family success and child welfare. Monitor and evaluate each child's care plan, making adjustments as their needs evolve. Support the recruitment, assessment, and training of foster parents. Establish collaborative relationships with foster parents to foster safe and nurturing environments for children. Arrange and coordinate medical, educational, and social services as required. Work closely with family assessment and planning teams to create coordinated service plans. Ensure compliance with documentation timelines as per internal policies. Adhere to health and safety standards set by

Health Connect America , State regulations, and Council on Accreditation. Resolve stakeholder issues promptly to avoid complications. Maintain high standards of work quality with a focus on ongoing improvement. Communicate effectively with all involved parties, keeping them informed and engaged. Promote team collaboration by welcoming feedback and contributing to overall efficiency. Qualifications: A Bachelor's degree in social work or a related human service field is required.
