Community of Hope is seeking a Training and Outreach Specialist, HONEY (Housing our Newborns, Empowering You) to educate, engage, and build relationships with individuals and families who are experiencing homelessness and are pregnant, connecting them with appropriate and preferred support systems and community resources.

This position is split between our Family Health and Birth Center in NE, Washington DC.

What You'll Do

During initial intake, assesses client's connections or barriers to receiving perinatal services, preferences for care, risk factors, and works to opt in client to HONEY perinatal care coordination. Identifies and links participants to appropriate and preferred community resources, perinatal care, and wraparound supports for family, in coordination with assigned housing case manager. Opts in, schedules, and sends rides for patients through Rides for Moms partnership with Uber Health (~ 30 rides per month). Document any relevant patient encounters in eCW within 3 business days of interaction. Updates referral and client tracking databases within 1 business day of interaction with clients. Conducts outreach activities to develop relationships with HONEY program participants and partnerships with other government and community-based organizations. Conducts outreach to homeless service and healthcare providers to build partnerships and opportunities for collaboration, training, and client support (e.g.: referrals, programming updates, etc.). Attends community events, tabling events, meetings, and conferences as requested to promote the HONEY program to potential clients and partners. Educates partners, clinicians and housing system staff on issues of perinatal health to support families during pregnancy and infancy. Conducts program overviews and educational sessions virtually, in person, and asynchronously for healthcare and homeless service providers to better support their work with pregnant people experiencing homelessness (approximately 2-4 sessions per month). Consults with COH Maternal & Child Health team and Housing team members in the development of presentations and training materials. Requirements

Must-Haves

Bachelor's Degree. Prior instructional experience. Strong organizational, written, and verbal communication skills. Strong interpersonal skills and comfortability in conducting cold calls and outreach. Basic graphic design, marketing, and presentation skills required. Must have reliable transportation. Nice-to-Haves

Experience in adult education, social services, health care, or nursing. Experience working with under resourced populations, including pregnant people experiencing homelessness. Why You'll Love Working Here!
