Children's Service Center
Purpose of this position:
The Hospital Liaison will assist with coordinating services from inpatient settings such as but not limited to inpatient psychiatric facilities, medical facilities, inpatient rehab, etc. The transitional therapist will be essential with ensuring intake into services is completed and will then assist with scheduling needed services. In addition, this therapist will follow the client to provide support, safety checks and therapeutic intervention from the time of inpatient discharge, to intake and continuing until a primary outpatient therapist session occurs. The transitional therapist will also provide therapy sessions as needed following inpatient discharge to the next appointment with the primary therapist if seven-day access from discharge to primary provider appointment cannot occur.

Qualifications
:
Bachelor's degree in social work or related field from an accredited school Must possess training and experience that has achieved recognition as a mental health worker. Master's Degree/ Licensed preferred
