Targeted Case Manager

Role Overview
Under the general direction of the Program Director, the Targeted Case Manager supports wellness and recovery goals by implementing targeted interventions to provide timely, high‑quality and efficient care. The TCM provides access to services that encourage clients to resolve problems interfering with their independence or self‑sufficiency and maintain themselves in the community.
Responsibilities
Provides case management to clients who are Health Home eligible, adults diagnosed with severe mental illness, adults with two SUD services within a year, and adults incarcerated more than 30 days in the past 12 months.
Provides individual counseling and facilitates case load group meetings and other assigned groups.
Supports improved community service linkages and referral to community and social support services.
Provides transitional care, support in navigating complex health care and social services system.
Provides individual and family support for clients and advocates for needed services.
Performs on‑site crisis intervention and skills teaching when other services are not provided.
Monitors that the services in the treatment plan are adequately addressing individual needs and wellness and recovery goals.
Monitors and follows up by contacting the service recipient, their family and collaterals, service providers, and other individuals or entities.
Qualifications
Bachelor’s Degree in Human Services required.
Master’s in Social Work preferred.
Bilingual Spanish (preferred).
Minimum 2+ years of experience in counseling severe clients with substance use disorders and/or mental illness.
