Position Summary – RN Hospice Case Manager Responsible for providing case management & discharge planning services for patients in a home health/hospice setting.
Responsibilities
Coordinate care within the continuum of care.
Develop patient-centered care plans.
Plan and manage discharge processes.
Collaborate with home health and hospice teams.
Minimum Required Qualifications
VT/compact RN license.
BLS certification.
1 year of hospice case‑management experience in a home setting.
Preferred Qualifications
Experience with rural travel.
McKesson/Netsmart EMR experience.
No RTO requests for this contract.
Assignment Details
Length of assignment: 20 weeks.
Shift: Sunday‑Thursday (first week M‑F for orientation).
Systems: McKesson/Netsmart.
Start date: ASAP.
Pay & Benefits
Pay rate: $1,800.00 – $2,700.00.
