Part-Time Case Manager
The Salvation Army Intermountain Division is seeking a dedicated Part-Time Case Manager to provide case management services to guests of temporary housing.
Responsibilities
Recruit guests and engage them in case management activities.
Conduct intake and create individualized case plans.
Deliver housing case management services aimed at self‑sufficiency and stability.
Perform assessments including Herth Hope Index, Self Sufficiency Matrix, and Coordinated Entry.
Manage data in HMIS, Coordinated Entry, WellSky, and other required platforms.
Organize and maintain confidential case files in compliance with HIPAA.
Assist program staff with household needs and community outreach.
Participate in case conferencing, team meetings, and required trainings.
Ensure compliance with funding contract requirements and organizational policies.
Qualifications
Bachelor’s degree in human services or related field. Equivalent experience accepted on a case‑by‑case basis.
Prior case management experience with families and/or unhoused individuals preferred.
Valid in‑state driver’s license and ability to complete annual MVR checks.
Ability to operate a vehicle safely and travel within the Denver metro area.
Strong written and verbal communication skills.
Capacity to work independently with limited supervision; some weekend hours may be required.
Physical ability to lift 25 pounds and perform necessary telephonic and computer‑based tasks.
