Veterans Community Health Liaison - New Mexico ALB
The Veterans Community Health Liaison (VCHL) strengthens the connection between veterans, their families, and the New Mexico Solutions Certified Community Behavioral Health Clinic (CCBHC). The VCHL promotes wellness and behavioral health awareness among veterans through targeted outreach, education, and advocacy. The position focuses on ensuring that veterans understand and access CCBHC services, receive coordinated care, and are connected to community and VA-based resources that support recovery and well-being. Outreach & Marketing
Represent the CCBHC in veteran and military community settings, providing information about available behavioral health and wellness programs. Develop and distribute marketing materials and social media content highlighting CCBHC services for veterans. Conduct community presentations and workshops at veteran organizations, resource fairs, and local events. Partnership & Collaboration
Build strong partnerships with VA medical centers, Vet Centers, and veterans’ service organizations in the Albuquerque region. Collaborate with CCBHC clinical and outreach teams to enhance services tailored to veterans’ needs. Support coordination of care between the CCBHC and external providers. Client Support & Navigation
Assist veterans and their families in accessing behavioral health services, case management, housing, and other resources. Provide follow‑up support and advocacy to ensure successful engagement in treatment and recovery programs. Identify and address barriers to care, advocating for veterans’ access to culturally competent services. Health Education & Prevention
Conduct health education on veteran‑specific topics such as PTSD, trauma recovery, suicide prevention, and substance use. Coordinate veteran wellness activities, peer‑led groups, and prevention campaigns. Ensure all educational materials and approaches are sensitive to military and veteran culture. Documentation & Data Collection
Maintain accurate documentation of outreach, referrals, and outcomes in the CCBHC system. Track data for grant and quality improvement reporting. Participate in evaluation of outreach effectiveness and program development. Qualifications
High school diploma or GED required; bachelor’s degree in human services, psychology, or related field preferred. Veteran or immediate family member of a veteran strongly preferred. Minimum two years’ experience in outreach, case management, or veteran support services. Must obtain BLS certification within six months of hire. Valid New Mexico driver’s license and reliable transportation required. Knowledge, Skills, and Abilities
Knowledge of veteran culture, military systems, and behavioral health challenges facing veterans. Strong communication, public speaking, and community engagement skills. Familiarity with VA and community-based resources available to veterans. Ability to collect, organize, and report outreach and service data accurately. Proficiency in Microsoft Office and electronic data systems.
