Qualifications
Age at least 21 years.
High school diploma or GED.
Valid driver’s license.
Reliable transportation and meet requirements for the Millcreek Motor Vehicle Safety Program if required to drive.
Willingness to work a flexible schedule.
Complete the HCBS training and continued education program.
Essential Job Functions
Provide support and information necessary to help participants make informed choices and decisions.
Establish collaborative relationships with participants, co‑workers, and others important to the participant.
Determine participants’ needs, desires, and interests.
Assist participants in identifying available support within their community, family, and social network.
Work with participants to develop and sustain relationships with friends, family, and community members.
Assist participants in managing everyday aspects of life by teaching and providing necessary supports.
Provide support to engage in satisfactory work and daily activities.
Complete necessary documentation efficiently and timely.
Perform routine daily checks of vans and report deficiencies.
Assist in plan development for participants, reporting to supervisor in a timely manner.
Assist participants in achieving outcomes identified in their plan.
Keep supervisor abreast of pertinent participant information.
Ensure participant safety at all times.
Report risk‑management issues to supervisor and submit timely reports.
Adhere to UltiPro usage, policies, procedures, and timelines.
Provide safe transportation to and from community activities.
Adhere to all Millcreek and HCBS waiver program policies and procedures.
Follow all safety policies and worker’s compensation program guidelines.
Other duties as assigned.
