Wage Range:

$25.00‑$29.00 per hour (non‑exempt, hourly)

Position Overview The Homeless Outreach Specialist works independently and collaboratively with a team of health and social service professionals to assist in the development and implementation of outreach services for homeless Veterans. The role serves as a bridge between the community and the system of housing, health, and social services for Veterans. Outreach efforts are conducted primarily at VA centers, shelters, and locations where homeless veterans may be gathered (encampments, meal programs, etc.). The specialist assists homeless veterans and their families to access needed services, including housing, behavioral healthcare, medical, and dental care. Hours are flexible to meet program needs with some evenings and weekends. The specialist uses trauma‑informed care and motivational interviewing techniques to engage Veterans struggling to access services due to complex psychosocial issues and cultural barriers.

What You Will Do

Participate in outreach activities to engage homeless Veterans and assess individual and family needs.

Perform brief screening and assessment when appropriate; assist veterans to access resources through the Homeless Veterans Registry and other coordinated access systems.

Engage clients by eliminating social, cultural, linguistic, and educational barriers and developing culturally competent action plans.

Coordinate with Veterans service providers, including the VA’s Community Resource and Referral Center and Transition Projects, Inc., and attend regular provider meetings.

Coordinate with community alcohol and drug treatment, detox, residential, and outpatient providers, as well as behavioral health providers.

Facilitate communication and positive relationship‑building between clients, veterans’ service providers, and other medical, behavioral health, and social service providers; guide clients through complex service systems.

Consult with clinical supervisor on complex cases exceeding area of expertise.

Apply a person‑centered approach, respecting self‑determination and exploring ambivalence using Motivational Interviewing principles.

Participate in agency and care team meetings to coordinate outreach activities as needed.

Respond to outreach requests from individuals in the community.

Collaborate with housing and auxiliary providers to develop care plans and goals.

Adhere to Trauma‑Informed Care principles and educate providers on this model as appropriate.

Advocate for timely access to appropriate services, promoting and modeling advocacy skills.

Coordinate patient transportation and accompaniment as needed.
