Children with medical homes elsewhere come to La Rabida for specialty services. La Rabida offers a wide range of specialty services provided to children with sickle cell disease, diabetes, and many others. Children are supported in their emotional and developmental growth, particularly in cases where such growth has been interrupted by accident or disease.
Finally, La Rabida provides forensic and treatment services for children exposed to abuse and neglect, comprehensive assessments for wards of the state, early intervention for children between 0 and 3 years of age, and care coordination services for medically complex children for those who are covered by a health plan and receive care from providers in Cook County.
Job Description The registry pediatric pharmacist provides clinical and operational pharmacy services on an as‑needed basis to support pediatric patient care across inpatient and outpatient settings. This flexible, per diem role requires a pharmacist with pediatric experience and strong clinical judgment to contribute to safe and effective medication use. The pharmacist will support admission and discharge medication coordination, outpatient services, and clinical interventions during variable shifts including weekends, evenings, and holidays.
Clinical Responsibilities
Review and verify medication orders for pediatric patients, ensuring safe, effective, and evidence‑based use.
Provide age‑ and weight‑appropriate dosing recommendations and drug monitoring.
Identify and resolve medication‑related issues and provide drug information to healthcare professionals.
Admission and Discharge Coordination
Conduct medication reconciliation during patient admissions.
Review and counsel on discharge medication plans to ensure clarity, safety, and accessibility.
Educate patients, families, and caregivers on medication administration and potential side effects.
Outpatient and Transitional Care
Collaborate with ambulatory care providers to resolve drug access, adherence, and authorization issues.
Assist with continuity of care across transitions between hospital and home or outpatient settings.
Operational Duties
Verify and oversee pediatric compounding, including sterile and non‑sterile preparations.
Assist with inventory control and ensure proper handling of pediatric‑specific medications.
Collaborate with technicians to ensure timely and accurate medication distribution.
Regulatory and Safety
Adhere to all hospital policies, regulatory guidelines, and standards of practice (e.g., Joint Commission, USP 795, 797, and 800).
Report and document medication errors, adverse drug events, and clinical interventions.
Promote a culture of safety in pediatric medication use.
Qualifications
Pediatric experience in a hospital or clinical setting.
Completion of PGY1 Pharmacy Residency preferred.
Pediatric inpatient and outpatient experience.
