Job Overview
SCO Family of Services has provided vital human services throughout New York City and Long Island for more than 100 years. We help vulnerable New Yorkers build a strong foundation for the future by supporting young children, youth, families, and adults with special needs.
Position Purpose The Care Manager services youth and adults enrolled in Medicaid, with conditions such as HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI), or Complex Trauma (ages 0‑21 for Children’s Program; 18 to end of life for Adult Program). The role coordinates, integrates, and partners in delivering comprehensive care to these populations.
Responsibilities
Obtain required Care Management enrollment consents and process enrollment into the Care Management Program.
Complete initial and ongoing needs assessments (e.g., Comprehensive Assessment, CANS‑NY, NYS Eligibility Assessment for HARP).
Develop and manage individual Plans of Care (POC).
Coordinate member services, ensuring adherence to treatment recommendations and facilitating referrals.
Monitor and evaluate member needs in Prevention, Wellness, Medical, Mental Health, Care Transitions, and Social Services.
Meet standard billing requirements based on acuity scores and maintain blended caseloads (High, Medium, Low).
Provide face‑to‑face contacts per department mandates and maintain timely, accurate documentation in the electronic health record.
Advocate for members and coordinate with agency and external providers.
Promote wellness and prevention by linking youth to appropriate resources.
Educate members/caregivers on chronic conditions, immunizations, screenings, and preventive interventions.
Assist in obtaining and maintaining public benefits (Medicaid, Cash Assistance, SS, SNAP, Housing, Legal Services, Employment, Training).
Effectively communicate with families and other caregivers, respecting language, literacy, and cultural preferences.
Conduct care planning meetings and collaborate with the interdisciplinary team.
Identify community resources and manage referrals, engagement, and follow‑up.
Engage in discharge planning for hospital admissions and ensure post‑discharge services.
Participate in ongoing staff development training.
Ensure members receive periodic evaluations for Dental, Vision, and Hearing care per Medicaid EPSDT guidelines.
Maintain close working relationships with case planners or case managers.
Travel within the five boroughs, Nassau, and Suffolk counties as needed.
Perform other duties as assigned.
Qualifications
Bilingual preferred.
Health Home Care Manager candidates: for children with high acuity (CANS‑NY) or adults with Health Home Plus status – Bachelor’s degree + 2 years relevant experience, Registered Nurse license + 2 years, or Master’s degree + 1 year.
Health Home Care Manager for children with medium acuity (CANS‑NY): Associate’s degree + 1 year relevant experience.
Expertise in child welfare, developmental disabilities, mental health, health care, or related systems; advocacy experience.
Passion for supporting staff, children, adults, and families.
Desired Characteristics
