As a Nurse Care Manager, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.

Job Duties Collaborate with members and their families to maintain the most independent living situation possible. Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status. Authorize covered services and coordinate care across different payers. Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.
Experience & Skills Required Current NYS RN License and registration Associate's Degree in Nursing Required. Bachelor of Science in Nursing (BSN) Preferred Minimum of 3 years of RN experience Clinical expertise in geriatrics preferred; managed long-term care experience is also a plus. Bilingual Spanish or Chinese preferred
Other Information Salary Range:
105,000 - $115,000 Bonus:
