The
Care Coordinator
is responsible for: Providing an exciting opportunity to work within a community-based, integrated care model. Working as part of a multi-disciplinary team and will assist Mass Health recipients navigate available community resources to address and improve their overall healthcare needs. Working in a community setting, meeting members in their homes, shelters, and other relevant locations, and working on goals that support overall health care needs. Conducting outreach to members to enroll and keep engaged in the community partner program. Providing care coordination to enrolled members to address priority needs including housing, nutrition, mental/physical health, and substance use recovery. Completing comprehensive assessments, person centered treatment plans and all related documentation within established timelines. Facilitating collaboration with community services and collateral agencies that may support a member's expressed goals.
Qualifications
Qualifications: BA/BS degree in a field related to human services with a minimum of 1 year of experience; OR High School diploma/GED with a minimum of 3 years of experience working with adults in community-based and/or medical settings. Experience collaborating with individuals with substance use problems and mental illness. Knowledge of community-based resources that address social determinants of health. Satisfactory understanding of HIPAA laws and capacity to use an electronic health record. Competence in serving culturally diverse populations. Excellent communication skills and ability to work effectively within a team environment. Must hold a valid drivers' license. Must have access to an operational and insured vehicle. Spanish speaking/bi-lingual preferred but not required.
