Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager provides intensive case management and independent living skills development to foster youth and probation youth transitioning into independent living. The role assists youth in developing skills, obtaining employment, pursuing educational goals, and creating life plans that support independence.
Services are client‑centered, trauma‑informed, and culturally sensitive, consistent with Lifeline’s philosophy and mission to build self‑sufficiency. The Case Manager possesses knowledge and expertise related to transitional‑age youth, specifically former foster youth, and demonstrates excellent engagement skills.
Key services include life skills training, meditation, economic literacy, transportation assistance, public benefits advocacy, financial assistance, tenancy training, and supportive services for parenting youth (individualized housing, childcare assistance, education support, maternity/paternity provisions, and child services).
The role provides linkage to community services to support transition to independence and aftercare, including mental health services, educational and employment support, crisis management, and safety planning.
Case management involves a therapeutic component; evidence‑based practices should be utilized. Participation in county collaborative meetings to identify service gaps, barriers, and successes is required, as well as training, outreach, and partnership development with other agencies.
Qualifications
BA in social work or related field required
Masters and BBS registered intern preferred
Minimum of 3 years relevant experience required
Experience with teens and/or young adults, especially former foster and probation youth preferred
Specific Activities Duties include but are not limited to:
Utilize engagement strategies to build rapport, identify strengths and barriers to participant self‑sufficiency, and empower youth to achieve goals and permanent independence.
Provide intensive case management services designed to build and support participants’ relationships with family and community with an emphasis on permanent connection.
Utilize evidence‑based practices to engage youth in client‑centered, culturally appropriate, trauma‑informed services that improve short‑ and long‑term health, safety, and well‑being.
Apply evidence‑based practices for crisis management, safety planning, and individual/group sessions.
Coordinate care with mental health professionals, primary care providers, and other supportive services.
Engage youth in community activities to stimulate alternative interests, improve emotional and physical well‑being, foster leadership development, enhance self‑esteem, and support personal growth.
Provide support, advocacy, life skills, referrals, crisis intervention, and employment readiness.
Carry a caseload of youth as required by funding source and provide responsive services in office, home, and community settings.
Outreach and marketing to prospective participants and community partners.
Work closely with the Housing Specialist to screen and interview prospective participants.
Assist youth in obtaining documentation, housing, education, and legal services.
Assess applicants, co‑create individualized case plans, monitor the plan, and provide assistance as needed.
Process assistance requests for participants.
Maintain accurate, timely documentation for each client, tracking progress in Efforts to Outcomes (ETO) as required by contract.
Assist in program development and resource identification, maintaining linkages with community, educational, and employment resources.
Collaborate with County ILS workers, ILS contractors, social workers, probation officers, and other parties in service plan delivery.
Participate in multidisciplinary teams, serving as an expert on behavioral health matters.
Attend and participate in program meetings, group supervision, retreats, committees, trainings, and other collaborative activities.
Maintain confidentiality and privacy standards of participants’ protected health information in accordance with HIPAA.
Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants.
Work independently and use sound judgment in decision‑making.
Complete all other duties as assigned.
Schedule / Compensation
Full‑time, non‑exempt position
Mon‑Fri 8 hours a day may include evenings and weekends
Evening and weekend on‑call shifts
Salary: MA or BA +5‑[21.00‑26.64]; MA+BBS [26.64‑30.00]. Differential pay offered for bilingual skills. EEO.

Valid driver’s license and reliable transportation with a good driving record.
Willingness to use own insured vehicle for the job (mileage reimbursed).
Employment subject to clearances, including background fingerprint checks, driving record, tuberculosis testing, and drug testing.
