About This Position:
The Licensed Medical Social Worker plays a vital role in collaboration with a compassionate care team, delivering essential support services to patients and their families. This position is instrumental in ensuring that all staff members provide high-quality care throughout the patient's journey, from admission through discharge and into aftercare. Adherence to department-specific policies and procedures, as well as maintaining assigned competencies, is essential. Main Responsibilities: Contribute to department goals and objectives while adhering to policies, procedures, quality, and safety standards. Ensure compliance with governmental and accreditation regulations. Assess the psychosocial needs of patients and their families, particularly concerning terminal illnesses. Work collaboratively with the interdisciplinary team to develop and implement a comprehensive care plan. Document psychosocial assessments, goals, interventions, and patient family responses effectively. Conduct assessments and referrals for ongoing bereavement services. Support patients' unique cultural and spiritual beliefs by facilitating connections to clergy or chaplains as needed. Enhance the caregiving environment by linking patients, families, and caregivers with community resources. Provide interventions targeting specific symptoms such as fear, grief, depression, and anger. Screen for psychopathology and provide appropriate education and interventions. Qualifications: A Master’s degree in social work is required. Preferred experience in healthcare and home health care settings. Completion of a hospice training program is preferred. Licenses and Certifications: A valid LMSW (Licensed Medical Social Worker) certification from the state is required. This position offers a competitive hiring range of $29.15 - $48.99 per hour.
