Position Summary
We are seeking a dedicated and experienced Director of Nursing (DON) to lead the clinical operations of our growing home health agency in Indiana. The ideal candidate will have strong leadership skills, a deep understanding of OASIS documentation, and experience in compliance with CMS regulations and Indiana state requirements.

Key Responsibilities
Oversee day-to-day clinical operations and supervise all field and office clinical staff.
Ensure timely and accurate completion and submission of OASIS assessments, including SOC, ROC, Recertification, and Discharge.
Maintain compliance with federal, state (Indiana Department of Health), and accreditation standards.
Participate in the development and implementation of policies and procedures.
Conduct performance evaluations, training, and in-services for nursing and paraprofessional staff.
Review clinical documentation for accuracy, completeness, and compliance with Medicare and Medicaid regulations.
Cooperate with physicians, case managers, and other healthcare professionals to coordinate patient care.
Participate in quality assurance / performance improvement (QAPI) initiatives.
Serve as a clinical resource for staff and assist in complex clinical decision-making.
Manage staff schedules to ensure appropriate coverage and efficient service delivery.
Requirements
Registered Nurse (RN) licensed to practice in the state of Indiana (unencumbered license).
Minimum 2–3 years of DON or leadership experience in a home health setting.
Extensive OASIS experience and understanding of CMS regulations.
Strong knowledge of Medicare Conditions of Participation (CoPs) and Indiana state home health regulations.
Experience with EMR systems.
Excellent leadership, communication, and organizational skills.
Ability to work independently and manage multiple priorities.
Must pass background check and drug screening per agency and state policy.
Preferred Qualifications
BSN or MSN degree.
Experience with home health accreditation surveys (e.g., CHAP, ACHC, or Joint Commission).
QAPI and infection control program experience.
